SNAP (Supplemental Nutrition Assistance Program) is a county-administered, federal program that assists people and families with low incomes to purchase food and better meet their nutritional needs.
The program is not intended to supply all of a person’s or family’s food needs. Instead, it is a supplement to help them achieve or maintain their independence.
If you need assistance with your SNAP application, call Second Harvest Heartland Care Center at 866-844-3663 or 651-401-1411. They are available for one-on-one eligibility screening and application assistance.

Cold items available as well (salad, dessert, bread, butter and milk or juice).
Please dine with us. In an effort to help older adults live healthy, independent lives, we provide a nutritious midday meal to working, retired, and semi-retired persons age 60 and over at our congregate dining site at the Chisago County Community Center in North Branch and through our Meals on Wheels home-delivery program. The Senior LinkAge Line® is a free service in Minnesota that connects older Minnesotans and their families with the help they need. Assistance is provided over the phone or in person on a regular basis.
The Senior LinkAge Line provides free help to older adults who may need support remaining in or returning to their home. Providers can submit a referral or a person can self-refer for help. When we receive the referral, we will work with the person and/or caregiver to understand the person’s needs and preferences, help connect them with services in their community and follow-up to support long-term success.
The Senior LinkAge Line has free information you can share with your clients so they know who to call if they need support. We also offer interactive presentations to professional staff or your clients. Call the Senior LinkAge Line at 800-333-2433 to schedule a presentation on:
- Medicare
- Fraud prevention
- Preadmission screening
- How the Senior LinkAge Line can help
In addition, MinnesotaHelp.info is available to everyone to find local resources. This is the same resource database the Senior LinkAge Line staff and volunteers use to find and connect services and supports to people across the state.
We distribute food to nearly 1,000 partners in our 59-county service area. We also provide assistance with Federal Nutrition Programs, SNAP (Supplemental Nutrition Assistance Program) and NAPS (Nutritional Assistance Program for Seniors). We work to remove barriers and make benefits accessible to all eligible community members by providing program awareness, application assistance, and submission and follow-up during the process.
Every Thursday June through September, 3pm to 7pm.
