Minnesota Aging Pathways is a free service in Minnesota that connects older Minnesotans and their caregivers with the help they need. Assistance is provided over
the phone and via online chat on a regular basis.
Limited in-person support is available.
 
Minnesota Aging Pathways provides free help to older adults who may need support remaining in or returning to their home. Providers can submit a referral, or a person can
self-refer for help. When we receive a referral, we will work with the person and/or caregiver(s) to understand the person’s needs and preferences, help connect them with
services in their community, and follow-up to support long-term success.
 
Minnesota Aging Pathways has free information you can share with your clients, so they know who to call if they need support. We also offer interactive presentations to
professional staff and to the public.

Lakes & Pines Trained navigators are available to help you enroll through MNsure for health care coverage. Everything may be done in-person or over the phone and via computer. Even if you don’t have a computer or internet access, staff will be able to assist you.

Veterans Services are provided through Chisago County Health and Human Services on the main floor of the Government Center. We provide guidance and assistance to veterans, their spouse / survivors and dependents. We are here to help you apply for veterans benefits as provided by the Minnesota Department of Veterans Affairs and the United States Department of Veterans Affairs.

In addition, we provide transportation to the VA Health Care System – Minneapolis and the VA Health Care System – St. Cloud to those in need of this service. We also provide referrals to a variety of other public and private programs and work closely with veterans organizations concerning programs and changes.
