Our Goal- The Chisago Age Well Coalition strives to provide older adults and family caregivers with opportunities to age well in their community.

Our Mission

To increase awareness of, access to, and satisfaction with, existing services and health resources in the community; and to strengthen the relationships between senior community members, service providers, and health systems to benefit older adults and their caregivers

The Chisago Age Well Coalition is a recognized 501(c) (3) not-for-profit. We are a group of more than 30 local community members and service providers both private and not-for-profit, who work together to benefit older adults and their caregivers in Chisago County. Created in January 2018 as the result of a statewide initiative called Silos to Circles, 

In 2020 Chisago Age Well was recognized as being a Silo to Circles Community with an Achievement award from the National Association of Area Agency on Aging (N4A) 

Chisago Age Well is comprised of over 30 different agencies and local community members that support or provide services to older adults and their caregivers.
