The Chisago Age Well Coalition was awarded the Joy and Connections grant in August of 2025 through the Initiative Foundation.
Our mission is to increase awareness of, access to, and satisfaction with local services and health resources, and to strengthen relationships between older adults, caregivers,
service providers, and health systems. This grant will allow us to foster community connections, support local belonging, and strengthen community purpose through the following:
- Dine & Discovers, in partnership with North Branch Community Education provides informal events led by experts on key topics. 2026 topics: Self-Care as We Grow Older: Practical Pointers for Food and Exercise, Natural and Homeopathic Medicine: Pros and Cons and Alzheimer’s and Dementia Prevention and
Treatment: What’s New? - Age Well Expo, a free event with 60+ vendors and an opportunity to learn about community services, volunteer opportunities and activities for older adults. The
event also includes health screens, entertainment, food and a keynote presentation. Partnership with Chisago Lakes Area Community Education - Quick Look Directory printed guide to community resources for older adults.
- CAWC website and Facebook page will promote community events and resources hosted by local organizations and groups.
